AOD Case Manager - Venndale Rehabilitation Centre

The case manager is responsible for ensuring an effective, high-quality delivery of rehabilitation services to individual clients. This will involve building strong and influential relationships, for the purpose of providing alcohol and other drug (AOD) specialist support required to facilitate recovery and rehabilitation from (AOD) problematic use, including volatile substances and co-morbidity. Provided support is to be culturally sensitive and compliant with the structured program and service delivery agreements.

Key Responsibilities:
• Ensure client admission file is completed in accordance with regulations, including assessment and signed informed consent paperwork to access other relevant information.
• Work with clients to develop a personal Recovery plan.
• Monitor and record all client needs and progress, including daily appointments.
• Provide or facilitate both individual/ group therapy and counselling sessions.
• Coordinate client appointments, business and recreational activities / outings as required, including family visits and cultural activities
• Liaise with other stakeholders, agencies and service providers in a prompt and professional manner
• To develop an ethical and confidential working relationship with individual clients to provide the appropriate support
• Collaborate with the aftercare coordinator to develop a client aftercare plan.
• Participate in supervision and case reviews.
• Participate in planning and program reviews as required.
• Participate in quality assurance activities and statistical duties as required.
• Undertake other duties as required by management

Essential Competencies (knowledge, skills and attributes):
• Competent use of computer software packages such as word, excel, email and internet.
• Willingness to provide Criminal History Check
• Current NT Ochre Card
• Current NT Drivers Licence
• Ability to work in a team as well as unsupervised.
• Cross cultural awareness and effective communication skills
• Working knowledge of EEO principles and practices
• Working knowledge of WHS principles and practices
• Working knowledge of all Kalano Policies and Procedures
• Willingness to undertake further training as required.
• Fully COVID-19 Vaccinated
